 How to Become a Patient

Become a PatietnCity of Hope sees patients at all points in their care, from diagnosis, to treatment, to follow-up. You can request to become a City of Hope patient online or over the phone, learn how to schedule consultations and get information about insurance and financing.

To make an appointment for yourself, a family member or a friend, please contact City of Hope either by:

After quickly reviewing your information, a new patient specialist will call you to ask a few preliminary questions about the diagnosis, confirm insurance information and address any non-medical queries you may have. Once your eligibility is verified, a nurse will call you to schedule an appointment with a consulting physician, as well as arrange to obtain relevant medical records. Please complete the “Permission to Release Protected Healthcare Information to City of Hope” form to expedite the process.  Please fax completed form to New Patient Services at 626-471-9297.

 New Patient Services Office Hours

We are open Monday through Friday from 8 a.m. to 5 p.m. (PST). Calls received after 5 p.m. will be returned the following business day.

 Interpreter Services

City of Hope offers free interpretation for patients and caregivers whose first language is not English.

 Is My Insurance Accepted?

Your insurance company/medical group will tell you if you need any authorizations. You can also find out what, if any, co-payments and deductibles will be your responsibility. Click here for insurance information. 

NOTE: If you are enrolled with a HMO, your insurance will not pay for your health care at City of Hope unless you have an authorization. A written authorization, which gives you permission to be treated at City of Hope, must be obtained from your insurance company/medical group before each planned admission and each type of outpatient treatment, e.g. chemotherapy, ambulatory surgery, or diagnostic service.
