City of Hope sees patients at all points in their care, from diagnosis, to treatment, to follow-up. You can request to become a City of Hope patient online or over the phone, learn how to schedule consultations and get information about insurance and financing.

After quickly reviewing your information, a new patient specialist will call you to ask a few preliminary questions about the diagnosis, confirm insurance information and address any non-medical queries you may have. Once your eligibility is verified, a nurse will call you to schedule an appointment with a consulting physician, as well as arrange to obtain relevant medical records. Please complete the “Permission to Release Protected Healthcare Information to City of Hope” form to expedite the process.  Please fax completed form to New Patient Services at 626-471-9297.
